Regarding the requesting of the Civil Status official documents from Ukraine, persons living abroad apply to the diplomatic missions and consular posts of Ukraine in the countries of their residence to make a request to Ukraine. The relevant questionnaire for retrieving documents from Ukraine should be filled out and accompanied by the cover letter, copy of the passport/ID and a consular fee of 330 ZAR for the document or 830 ZAR for the legalized/apostillized document (an extra fee for legalization/apostillization applies, more at https://minjust.gov.ua/dep/ddr/poryadok-prostavlennya-apostilya).  

The questionnaire must be filled out exclusively in Ukrainian in 3 copies, for each document separately. The answers to the questions should be complete and accurate.

A request to the archival institutions of Ukraine could be made independently by sending to the address of the relevant Ukrainian archive a completed questionnaire of the established sample to request a document from Ukraine. 

Diplomatic missions and consular authorities of Ukraine shall inform the person who requested the document from Ukraine about the results of the request made by a person or by the diplomatic mission/consular institution. (the processing time of such sort of requests could take from a few months to one year or even more due to the currently limited diplomatic mail delivery).
